85 of Cluj county’s 104 localities have at most 2 registered doctors with a place of work — a sign that access to specialist doctors is, outside a few towns, very limited. The only specialty that covers almost the entire territory is family medicine, present in 100 localities. At the opposite end, paediatrics exists in just 8. How concentrated the medical corps is
The starting point is the geographic imbalance. Of the county’s 5,832 doctors, 3,706 (63.5%) practise in Cluj-Napoca, while the remaining 2,126 are spread across all the other localities. The consequence is seen directly at the level of communes and small towns:
- 85 of 104 localities have at most 2 doctors in total;
- the critical mass of specialists is found only in a few urban centres;
- for much of the county, “going to a specialist” means “going to town”.
This is the definition of a medical desert: not the total absence of doctors, but the absence of an offer large enough for a patient to find, close to home, the type of doctor they need.

The clearest way to measure the deserts is to count in how many localities there is at least one doctor of a given specialty. Of the 104 localities:

The difference is enormous. Family medicine reaches 100 localities, while all the other specialties above remain concentrated in at most 11. In practice, beyond the family doctor, the access map empties out abruptly.
Family medicine: the safety net
The fact that family medicine is present in 100 of 104 localities is no accident — it is precisely its role in the system. The family doctor:
- is the entry gate to the rest of the specialties;
- issues the referral without which many specialist consultations are not covered;
- remains, in most communes, the only doctor available locally.

Paediatrics, the biggest desert
The figure that stands out the most is paediatrics: present in just 8 localities out of 104. For a family with children in a commune without a paediatrician, this means that specialist paediatric care almost certainly requires travel to a town.
It is important to nuance this: the absence of a paediatrician does not mean the absence of care for children. The family doctor, present in 100 localities, routinely cares for children too. The figure nevertheless clearly shows where the specialist offer is — and how rarely it appears outside the towns.
What these figures show and what they don’t
The data describe presence, not capacity:
- They show in how many localities there is at least one doctor of a given specialty.
- They do not show how many patients each doctor has, how busy a practice is, or how long you wait for an appointment.
- They do not show whether a doctor from a neighbouring commune covers, in practice, your locality too.
Even with these caveats, the overall picture is unequivocal: the county has a single dense centre, Cluj-Napoca, and a periphery where specialists are almost entirely missing.

The data come from the public register of the Cluj College of Physicians (Colegiul Medicilor Cluj, membrii.colmedcj.ro), in the form available on 3 July 2026. The register comprises 5,832 doctors, 104 specialties and 104 localities.
The “presence” of a specialty means that there is at least one doctor with that specialty registered with a place of work in the respective locality. A doctor who practises at several institutions is counted only once. The figures reflect administrative registrations, not real patient flows.
