A psychiatrist is a doctor and can be seen for free through CAS (the national health insurance house), and the most direct route to state services is the Centrul de Sănătate Mintală (CSM, Mental Health Centre) in your area. Psychology and psychotherapy sessions are covered only partially and in limited situations, so in practice many patients pay privately. Below you’ll see the administrative difference between the two specialists, how to reach them through CAS, and what to do in an emergency.

Psychiatrist vs. psychologist: who does what (administratively)

It helps to know who you’re dealing with, because the reimbursement rules differ:

  • The psychiatrist is a doctor specialised in psychiatry. They can make a diagnosis, prescribe treatment, and issue medical documents (a medical letter, sick leave, a referral). A psychiatrist consultation is a medical act covered by CAS.
  • The clinical psychologist / psychotherapist is not a doctor and does not prescribe medicines. They offer psychological assessment, counselling, and psychotherapy. In the public system, these services are covered on a limited basis, usually as ancillary services recommended by a doctor.

The two often work together, especially on the team of a Mental Health Centre.

How to reach a psychiatrist covered by CAS

For the psychiatrist consultation to be free (0 lei), you need three things:

  • to be insured (your insured status up to date);
  • possibly a referral, depending on the provider and the situation;
  • to go to a provider under contract with CAS.

About the referral: psychiatry is among the specialties with direct access in certain situations, which means you can sometimes reach a psychiatrist without a referral. At an outpatient psychiatry practice, however, a referral from the family doctor may be required. Because the rules in the framework contract do change, ask CAS or the practice before your appointment.

The Centrul de Sănătate Mintală (CSM): the most direct route

The simplest gateway into the public psychiatry services is the Centrul de Sănătate Mintală (CSM, Mental Health Centre). This is a community psychiatry service, usually organised within a hospital, with a multidisciplinary team: a psychiatrist, a psychologist, and a social worker.

  • At a CSM you can usually go directly for an assessment.
  • The services are usually free for the patient.
  • The CSM ensures continuity of care in the outpatient setting and the link with the family doctor.

CSMs operate under the Legea sănătății mintale nr. 487/2002 (Mental Health Law no. 487/2002), which governs care for people with mental disorders. Ask the Direcția de Sănătate Publică Cluj (Cluj Public Health Directorate) or the psychiatric hospital which CSM covers your area.

What is covered for psychology / psychotherapy sessions

Here expectations need calibrating. Clinical psychology and psychotherapy sessions are NOT unlimited and free through CAS. They can be covered as services ancillary to medical care, under a few conditions:

  • they must be recommended by a doctor (for example, by a psychiatrist);
  • they must fall within certain conditions provided for by the rules;
  • within a limited number of sessions per year.

Because the list of conditions and the number of covered sessions are set by CNAS rules and can change, check the exact situation with CAS or with the provider where you book. Outside these cases, psychotherapy sessions are usually paid for privately.
